The roles of privateand public health care and the paths of patient flow should also be considered.What is the hospital’s mission and strategy and are they likely to change? What are the political plans of the government forhealthcare and its financing? What future scenarios can you envision in the next 5, 10, 15 years?The environmental conditions within the hospital that should be considered include: The mission of the ICU, and the presence ofdifferent ICUs within the hospital.The clinical specialties that are available in the hospital, and the actual and expected patient mix for intensive care.Problems with nosocomial infections in the hospital.The organization structure of intensive care and other clinical services.The availability and organization of other High Dependency Care Units(other ICUs, intermediate-care or step-down units,postanesthesia care units).Sufficient availability of skilled staff.An interdisciplinary intensive care concept with concentrated resources and structure offers advantages that should beconsidered in the ICU design. At least following potential advantages can be foreseen:Efficient and controlled resource allocation; systematic application of best practice models; easier optimization of care processesand continuous presence of ICU professional staff.The core processes of the ICU depend on the characteristics of the hospital. Three common main horizontal processes can beused as the basis: patient care process, education, and optionally, research. The relative weight of these processes may haveconsequences for the unit design. In addition, support processes are essential. The most relevant for design are logistics,information technology, controlling, and administration - these should be adapted to the main processes.Health care has is confronted with increased uncertainties in the future. Therefore, ICU design should consider flexibility as anessential design goal at all levels.
